一位中年患者来“一笑堂”看病，德福刚要诊脉，被喜来乐制止了，患者十分奇怪，连说给你们送钱来了，你们干嘛不要呢？孟庆合来到“一笑楼”，想请喜来乐到他的药铺挂牌坐诊，当即遭到拒绝，孟庆和不甘心失败，以二十两银子要一个菜刁难喜来乐，喜来乐凭着自己的聪明才智将孟庆合戏弄了一番。为给喜来乐捧场，陈皮带了几十人来“一笑楼”吃饭，胡氏见自己忙不过来，叫跑堂的德福匆匆上了几个不干不净的现成菜，见食客们不悦，陈皮忙向大家讲述起喜郎中先前对众人的恩德，食客们连连称是。喜来乐从陈皮口中得知，赛西施与田魁翻脸后回沧州来了，心情顿时开朗了许多。德福拿来一个药方，认准师傅去祁州行医了，胡氏连连追问，玉儿看出了破绽，喜来乐觉得有人冒名顶替，决定去祁州查个究竟。路上，喜来乐师徒见到了被假喜来乐治死的病人，心中十分气愤。他让德福化装成孕妇，来到假“一笑堂”，当众揭露了假喜来乐的骗局。
